Referrals
Referrals may be made by physicians, patients or caregivers and patients may contact the clinic directly. Please fill out an Appointment Request Form found on this page and request a clinic appointment.
Before the first appointment is scheduled, prior medical records and other information gathered from the prospective patient and his/her physician must be evaluated. Examples of relevant records include laboratory data, medication lists, consultant reports and imaging studies. This comprehensive evaluation is necessary in order to determine if clinic visit followed by Next Generation whole genome or whole exome sequencing are likely to provide valuable clinical and diagnostic information. At the clinic patients will be seen by a physician with expertise in clinical genetics and a genetic counselor.
Sequencing is performed by a CLIA or CAP certified laboratory. Depending on the laboratory performing DNA analysis, a saliva or a blood sample will be required. Information gained from this advanced diagnostic tool is then used to inform diagnostic, treatment and medical follow up decisions. On-going management will be coordinated by the patient’s referring health care providers.
